Dolphin Supreme M5 not filtering dirt

Our Dolphin Supreme M5 puts dirt back into the pool. You can see the dirt coming form the water outlet on top of the unit as it moves through the pool. We carefully clean the filters after each use and they appear to be in good shape. The non-return flaps both seem to be working properly. Any suggestions?

The bag must have a hole in it or it's not seated on the bottom plate properly, for example, too loose.
